Passport Requirements

Passports Books and Passport Cards:
Customers may apply for a Passport Book, a Passport Card or both. The
Passport Card may only be used for land and sea travel between the United
States, Canada, Mexico, Bermuda, and the Caribbean. Passport Cards
are not valid for air travel.
Effective May 1, 2008,
Passport Services has notified acceptance agencies that all passport
applications must be submitted daily using the U.S. Postal Service.
In order to meet the pick-up deadline of the post office, the
Clerk of Court recommends that all passport applications be submitted by 3:00
p.m. Applications accepted between 3:00 and 4:00 p.m. will be mailed
the following business day and cannot be expedited that day.
FOR AGES 16 AND OVER: FORM DS-11
AGES 16 AND OLDER - PASSPORT VALID FOR 10 YEARS
The application
must be signed in our presence.
For ages 16 or 17, the child and parent must sign
the application.
If applicant is 16 or 17 years of age and has their own driver's
license or another official picture ID, they have to present it.
If they do not have one, the parent can use their ID.
For ages 18 and older, only the
applicant signs the
application. Applicant must
present their own identification.
Cost is check or money order made payable to U.S. Department of State
in the amount of $75.00 AND cash, check or money order made payable to the
Clerk of Court for processing application in the amount of $25.00. (Plus an
additional $10.00 if you wish to have your passport photo taken by the Clerk
of Court)
Must have two identical passport photos, which our office can take for
you.
Must have a certified copy of your birth certificate
or an old passport to send to Passport Services.
Bring
all of this into the Clerk of Court's Office for processing.
FOR AGES UNDER 16: FORM DS-11
AGES UNDER 16 - PASSPORT VALID FOR 5 YEARS (CAN NOT BE RENEWED)
The application
must be signed in our presence.
If the applicant is under
the age of 16, the child
and both parents must
sign the application.
Both parents must present their
driver's licenses or another official picture ID.
Cost is check or money order made payable to U.S. Department of State
in the amount of $60.00 AND cash, check or money order made payable to
the Clerk of Court for processing application in the amount of $25.00. (Plus an
additional $10.00 if you wish to have your passport photo taken by the Clerk
of Court)
Must have two identical passport photos, which our
office can take for you.
Must have a certified copy of child's birth certificate to send to Passport Services.
Bring all of this into the Clerk of Court's Office for processing.
RENEWAL: FORM DS-82
MUST HAVE BEEN 16 YEARS OF AGE OR OLDER WHEN ISSUED TO GET A RENEWAL
Cost is check or money order payable to U.S. Department
of State for $75.00
Must have two new passport photos (Clerk's Office will take for fee of
$10.00).
Must present old passport. (In order to renew, it
must be less than 15 years from the issue date printed on the passport.
If passport was issued more than 15 years ago, you must complete a new application DS-11.)
Mail above to the address supplied on the DS-82
form. Note: Address for standard delivery will
vary from address for expedited delivery.
Please note that Renewals and Amendments are not
processed by the Clerk of Court's office.
PASSPORT
CARDS
:
Passport Cards are not valid for air
travel.
Application
requirements for Passport Cards are identical to those for Passport
Books.
Validity: Ten years for ages 16 or older, five years
for ages under 16.
Cost for first time
applicants:
$20 for ages 16 or older (check or money order) payable to U.S. Dept. of
State;
$25 (cash or
check) payable to Clerk of Court for processing application, $10 (cash or
check) payable to Clerk
of Court for Passport
photos;
$10 for ages under 16
(check or money order) payable to U.S. Dept of State; $25 (cash or
check) payable to Clerk of Court for processing application,
$10 (cash or check) payable to Clerk of Court
for Passport photos.
