Passport Requirements
Please Note: St. Tammany Parish Clerk of Court will no longer be able to serve as a Passport Agent effective
May 1, 2011 due to recent changes and requirements for passport agents directed by the Bureau of Consular Affairs,
Passport Services, Customer Service Division, Washington D.C..
Our office will continue to accept passport applications at our Covington office through April 30, 2011.

Passports Books
and Passport Cards: Customers may
apply for a Passport Book, a Passport Card or both.
The Passport Card may only be used for land and sea travel between the United
States, Canada, Mexico, Bermuda, and the Caribbean. Passport Cards are not
valid for air travel.
Effective May
1, 2008, Passport Services notified acceptance agencies that all passport
applications must be submitted daily using the U.S. Postal Service.
In order to meet the pick-up deadline of the post office, the
Clerk of Court recommends that all passport applications be submitted by 3:00
p.m. Applications accepted between 3:00 and 4:00 p.m. will be mailed
the following business day and cannot be expedited that day.
Ages 16 and
over: Form DS-11
Ages 16 and older - passport is valid for 10 years. The application
must be signed in the presence of a Clerk employee. For ages
16 or 17, the child and parent must sign
the application. If applicant is 16 or 17 years of age and
has his/her own driver's license
or another official picture ID, he/she
must present it. If he/she does not have one, the parent
can use his/her ID. For ages 18 and older, only the
applicant signs the application. Applicant must present
their own form of identification.
Cost: $110 (check or money
order) payable to U.S. Department of State, $25 (check
or cash) payable to the Clerk of Court for processing
application, $10.00 (check or cash) payable to the Clerk of Court for two
required passport photos. Must
have a certified copy of your birth certificate or an old passport to send to
Passport Services.
Ages under 16: Form DS-11
Ages under 16 - passport is valid for 5 years and cannot be
renewed. The application must be signed in the presence of a
Clerk employee. When the applicant is under age 16, both parents must sign
the application. Both parents
must present their driver's licenses or another official picture ID
on behalf of the child. Cost: $80 (check or money order) payable to U.S.
Department of State, $25 (check or cash) payable to the Clerk of Court for
processing, $10 (check or cash) payable to Clerk of Court for two required
passport photos. Must have a certified copy
of child's birth certificate to send to Passport Services.
Passport
Cards : Application requirements for Passport
Cards are identical to those for Passport books. Validity: Ten years for ages
16 or older, five years for ages under 16.Passport Cards are not valid for air
travel.

Expediting a Passport
Application:
Expedite
fees are in addition to regular passport fees as stated
above.

Passport
Renewal: Form DS-82 and Passport
Amendment: Form DS-5504
Forms are available on
the Passport Agency website: www.travel.state.gov.
The Clerk's office may assist you with proper forms and new photos
as you prepare to submit your application directly to the Passport Agency.
Renewals and amendments are not witnessed, assembled, or mailed by the Clerk of
Court's office.
Adult passport must have been issued less than 15 years ago to
renew. If the passport was issued more than 15 years ago, applicant will
follow the instructions for Form DS-11. Applicant must have been 16 years
of age or older when passport was issued to renew. If applicant was
less than 16, follow the instructions for Form DS-11.
Two (2) required passport photos to accompany either form may be
taken at the Clerk's office for a fee of $10 (cash or check).
Renewal and amendment forms are filled out and signed by
applicants on their own.
Passport applicants are responsible for mailing completed forms,
along with old passport, new photos, and correct payment to the U.S. Passport
Agency. Mailing address is printed on the forms. Cost: $110 (check
or money order) payable to U.S. Department of State.
